One-on-One Case Management

Our case management services are tailored to meet your individual needs. Whether you're navigating a complex system or just need someone to listen and guide you, we're here for you. We can assist with:

  • Housing Support: Navigate the process of securing safe, stable housing, whether you're experiencing homelessness, dealing with unsafe living conditions, or need help understanding your rights as a tenant.

  • Name and Gender Marker Changes: Get help with the paperwork and process for updating your legal documents to reflect your identity.

  • Social Service Navigation: Apply for programs like food stamps (SNAP), childcare assistance, Medicaid, and housing support with confidence and ease.

  • Building Community: Connect with LGBTQIA+ social groups, support circles, and community organizations to reduce isolation and foster meaningful relationships.

What We Offer

Referrals

When you need support, knowing where to turn can feel overwhelming. We’re here to make it easier by connecting you with the right resources.
Our referral services include:

  • Gender-Affirming Healthcare Providers: From primary care doctors to specialists, we’ll help you find healthcare professionals who honor your identity.

  • PrEP and Sexual Health Resources: Connect with clinics that provide Pre-Exposure Prophylaxis (PrEP) to prevent HIV, along with other sexual health services.

  • Substance Use Support: Find programs and resources that prioritize harm reduction, compassion, and affirming care.

  • Gender-Affirming Clothing: Learn about resources for free or low-cost clothing that helps you express your authentic self, including chest binders, tucking garments, and more.

Peer-Led Harm
Reduction Support

Our harm reduction services offer a judgment-free zone where you can share your experiences and work toward your goals with the help of compassionate peers. We provide:

  • Substance Use Support: Discuss strategies to reduce harm, whether you're looking to quit, cut back, or simply use more safely.

  • Access to Resources: Get connected with harm reduction supplies, such as sharps containers and naloxone (Narcan), and learn about community programs that align with your needs.

  • Building a Safe Space: Join a supportive, understanding group that respects your experiences and provides encouragement as you navigate your journey.

  • Mental Health Support: Share openly in a peer-led environment and gain tools for managing stress, anxiety, and other challenges that impact your well-being.

Who Can Use These Services?

The Rainbow Bridging program is open to LGBTQIA+ residents of Rensselaer County. Services are always free, and while in-person support is prioritized, limited telehealth options are available on a case-by-case basis.

Why are we doing this?

At Collar City Pride, we believe everyone deserves access to affirming, inclusive care. We launched this program to bridge the gap between LGBTQIA+ individuals and the resources that empower them to lead healthy, fulfilling lives. With the support of our volunteers and the community, we’re building a stronger, safer, and more inclusive Rensselaer County.
